State of Florida Dementia Caregiver Survey

On behalf of the State of Florida, the Department of Elder Affairs, and the State Health Improvement Plan Priority Area 1, please share this survey with caregivers.

Florida has the second-highest prevalence of Alzheimer’s disease and related dementias (ADRD) with over 580,000 residents with a diagnosis. ADRD does not only affect the individual but also those who provide care for them and their family. In Florida, we have over 806,000 individuals providing unpaid care for their loved ones. This care is valued at over $ 20.6 billion dollars.

Florida’s numbers are continuing to grow and are predicted to increase by over 25 percent by 2025. Due to this reality, Florida is doing it all it can to be dementia capable.

We need your help with a very important survey we are working on. Your responses will help us to enhance support for those living with ADRD and their caregivers. The caregiver survey below is to determine the level of training being provided to caregivers, determine the needs of caregivers’ physical and mental health, the utilization of respite and emergency respite programs, and to increase the number of individuals participating in ADRD support groups, counseling programs and education platforms. We will not share your contact information outside of the Department of Elder Affairs, but we will let you know about available programs as we develop and release them.

State Dementia Survey

This self-assessment connects you to the TCARE (Tailored Caregiver Assessment and Referral) Program. This is a one-on-one caregiver support program that may require you to engage with an assigned TCARE Coach. If you wish to participate in the program, continue to the self-assessment. Please be advised that a TCARE Coach may be in contact with you via phone or email.
