New Program Announcement: GUIDE for Caregivers

Elder Options employee speaking to clients about caregiver support

We’re excited to share that Elder Options is now a proud participant in Medicare’s new GUIDE program, Guiding an Improved Dementia Experience. This innovative model offers free support to people living with dementia and their caregivers, including 24/7 access to a support hotline, care coordination, caregiver education, and short-term respite care.

We are currently providing GUIDE services in Alachua, Citrus, Dixie, Gilchrist, Levy, Marion, Sumter, Hernando, Lake, and Putnam counties, with plans to expand to the remaining counties in our 16-county region.

If you’re a caregiver needing a break or someone seeking help navigating dementia care, GUIDE is here to make the journey easier. We’ll also be hosting a monthly Caregiver Compass: Helping Dementia Caregivers Navigate Resources, where you can learn more about GUIDE and other Caregiver programs available at Elder Options. To register, click  HERE

If you or a loved one has Traditional Medicare (Parts A & B), you may qualify for GUIDE. It’s completely free to participate. If you are interested in learning more, please fill out the online form, call the Elder Helpline at 1-800-262-2243 or email guide@agingresources.org to be connected with a GUIDE Specialist.

This self-assessment connects you to the TCARE (Tailored Caregiver Assessment and Referral) Program. This is a one-on-one caregiver support program that may require you to engage with an assigned TCARE Coach. If you wish to participate in the program, continue to the self-assessment. Please be advised that a TCARE Coach may be in contact with you via phone or email.
